Prolotherapy:

The Foundation of Regenerative Injection Techniques

What Is Prolotherapy?

Prolotherapy—short for proliferative therapy—is a regenerative injection technique that helps stimulate the body’s natural repair process to strengthen and stabilize tendons, ligaments, and joints.

It is often the first-line treatment for recent sprains, strains, and smaller connective-tissue injuries—and serves as the foundation of all regenerative injection techniques offered at Maui Regenerative Medicine.

How It Works

Ligaments and tendons can become stretched or partially torn from injury, overuse, or age-related changes. These tissues have limited blood supply, which can slow healing and lead to chronic pain or instability.

Prolotherapy involves precise, ultrasound-guided injections of a mild proliferant solution (often dextrose-based) into these weakened attachment points. The solution triggers a controlled local healing response, encouraging the body to strengthen and rebuild connective tissue.

The goal: restore joint stability, reduce pain, and support long-term function.

Conditions Commonly Supported by Prolotherapy

  • Ligament or tendon sprains and strains
  • Chronic musculoskeletal pain
  • Mild to moderate arthritis
  • Whiplash and neck pain
  • Lower back pain and sacroiliac joint instability
  • Shoulder, elbow, hip, knee, and ankle instability
  • Plantar fasciitis and foot pain
  • Repetitive stress or overuse injuries

What are Clients saying?

Why Choose Prolotherapy?

  • Non-surgical and natural – stimulates your own body’s healing response
  • Targets the source of pain – ligament and tendon instability, not just symptoms
  • Improves function – helps stabilize joints for long-term relief
  • Complementary to PRP and Orthobiologics – often used as a first or foundational phase before advanced regenerative treatments

Prolotherapy vs. Cortisone

While cortisone injections temporarily reduce inflammation, they can also weaken collagen tissue over time.
Prolotherapy takes the opposite approach—it encourages collagen repair and tissue strengthening for long-term joint stability.

What to Expect

Step 1: Comprehensive Evaluation
Dr. Davison performs a detailed orthopedic exam, including ultrasound imaging, to pinpoint areas of ligament or tendon weakness.

Step 2: Targeted Injections
After local anesthetic, a proliferant solution is injected at precise attachment points where connective tissue is compromised.

Step 3: Recovery & Strengthening
Mild soreness may occur for a few days as the healing response begins. Over the next 4–6 weeks, new collagen fibers strengthen the treated tissue.

Most patients require 3–5 sessions, spaced 2–4 weeks apart, depending on injury severity and health status.

Why Early Diagnosis Matters

Many patients experience chronic pain because ligament laxity or small connective-tissue tears go undetected on MRI or X-ray.
Dr. Davison’s expertise in diagnostic ultrasound allows for real-time visualization of these micro-injuries—ensuring treatment is directed exactly where it’s needed.

A Brief History of Prolotherapy

Originally developed in the 1940s by Dr. George Hackett, Prolotherapy has been used successfully for decades in orthopedic and sports medicine.
Multiple studies have demonstrated improvements in pain, stability, and tissue strength, confirming what clinicians have long observed: Prolotherapy helps restore function naturally.

Patient Experience & Outcomes

Most patients begin noticing improvement after the first few sessions as joint stability increases and pain diminishes.
Relief typically continues to build for several months as the tissue remodels and strengthens.

“It’s one of the most practical and foundational regenerative tools we use. For many, it’s the bridge between short-term relief and true recovery.”
Dr. Kevin Davison

Prolotherapy at Maui Regenerative Medicine

Dr. Davison has practiced regenerative medicine for over 25 years and was among the first in Hawaiʻi to integrate ultrasound-guided Prolotherapy, PRP, and Orthobiologic therapies.
Each treatment plan is customized to the patient’s injury, health status, and activity goals—helping individuals heal safely, stay active, and avoid unnecessary surgery.
